APPLICATION INSTRUCTIONS
Applicants must submit a completed GRANT APPLICATION FORM in applying for a grant. Applications must be delivered via postal service (or another delivery service), email, or through the Foundation’s website portal by the following dates. The Foundation’s mail address, email address, and website address are provided on the GRANT APPLICATION FORM.

Grant applications must contain:

A completed GRANT APPLICATION FORM.

ATTACHMENTS listed on the grant application form.

3 LETTERS OF REFERENCE for requests over $10,000.
Incomplete application packages will not be considered UNLESS the applicant submits an acceptable explanation for the deficiency in writing and accompanying the package.

Optionally, applications may be printed and delivered via the postal service.
All applications are limited to 20 pages, not including required attachments.

LETTERS OF REFERENCE
Grant applications requesting over $10,000 must include 3 letters of reference. Download our Letter of Reference Form and send it to 3 individuals who can serve as your references. Direct your references to send their letter to info@koskovichfoundation.org. (**Those who have received funds from our foundation in the past can skip this step.)

FREQUENCY OF APPLICATION
Grants are reviewed 2 x per year. If your application is rejected, you may resubmit your application for the same project in one year. If the rejection is due to a lack of information, you may resubmit in the next application cycle. You may apply for a new project at any time.
REPORTING REQUIREMENTS
Applicants who receive funds from the Foundation must agree to provide reporting. Such required reporting may be tailored by the Foundation’s board to the project.
AWARDING OF GRANTS
Applicants will be notified of grant award determination within 30 days after the Grant Committee considers the applications received.
